CARE AT HOME

The Care at Home tender seeks a specialised service provider to deliver proactive, person‑centred support within the homes and communities of residents in the Metropolitan Borough of Bury. The aim is to reduce, prevent or delay the need for additional care, promote statutory individual wellbeing, and foster positive behavioural change that encourages as much independence as possible. The outset of care is tailored to each individual’s assessed needs and outcomes, with a focus on preventing crisis situations and enhancing overall quality of life.

Procuring the service is divided into six geographically specific lots: North, West, East, Whitefield, Prestwich, and Borough Wide provisions. Each lot is awarded on a five‑year contract with the possibility of two additional twelve‑month extensions, reflecting the department’s commitment to long‑term service continuity. The contractor will collaborate closely with local health and social care teams to ensure seamless integration of support across the borough.

All proposals will be assessed solely on quality, requiring evidence of a consistent person‑centred approach, responsiveness to changing needs, and the provision of strategies that nurture independence. All service delivery must meet nationally recognised quality standards for community care.

Description of service
The main aim of the Care at Home service is to provide proactive and personalised care within the individual's home and community. This means that the service supplier should work in a person-centred way and respond to a person's changing needs to preventing crisis situations from occurring. This will address the person's social-care related quality of life as well as their wider wellbeing in line with their assessed needs and individual outcomes.
