Description

Sheffield City Council is establishing an Enhanced Care Framework for people over the age of 16 years old who have learning disabilities and/or autistic people, who display behaviours that challenge including those with a mental health condition. This Enhanced Care Framework will encompass a range of services as outlined in the specific Lots and sub-Lots: Lot 1- Enhanced supported living Lot 1.1 – Support linked with accommodation Lot 1.2- Community care and support Lot 2- Severe Multiple Disadvantage Lot 2.1- Support linked with accommodation Lot 2.2- Community care and support Lot 3- Care and Nursing Home Lot 3.1 - Residential Care - Care home services without nursing Lot 3.2 - Nursing Care - Care home services with nursing Lot 4 - Planned Enhanced Overnight Short Breaks Lot 5 - Emergency Enhanced Overnight Short Breaks Lot 6 - Crisis/Bridging Support This Framework Agreement has been procured as an open framework. Open frameworks have a maximum duration of eight years. Within the eight-year term, the framework must be reopened at least twice. The Council will open the Framework after the initial three years from the day of the first award. The Council’s intention is that Framework two will then reopen after three years, Framework three will then have a duration of two years. Providers will meet the shared objectives of delivering support which meets the needs and outcomes of the individuals, as well as improving choice, quality and promoting independence. The Council is looking to shift towards outcomes-based commissioning. In its broadest terms, this means that the focus will be moved from ‘how’ a service operates, to the outcomes the service achieves. This means that the individual and/or their advocates will be involved in measuring the success of a given service in relation to achieving outcomes, and their role will be more central to the evaluation and management of contracts.

Description: This Lot is for people whose primary diagnosis is a learning disability and/or autism. The people who are experiencing severe multiple disadvantage present with a complex mix of needs and risks, which may include but not limited to: ADHD, and acquired brain injuries Mental health conditions Physical health conditions Histories of trauma, abuse, and significant adverse childhood experiences History of or current substance use High levels of vulnerability to exploitation, cuckooing, and abuse Offending histories, including arson, assault, and sexual offences Social isolation, poverty, poor family support, and histories of homelessness People who experience severe multiple disadvantage often have presenting conditions that are undiagnosed due to the difficulties this cohort faces in accessing these services. It is anticipated that most of the people who will come via this Framework will not require any CQC regulated activity. Although it would be preferable, for this specific Lot, providers do not have to be CQC registered.

Description: The Framework for enhanced Residential Care and Nursing Homes will replace spot contracting arrangements for specialist placements for people described in the overarching specification who have a learning disability, autism and/or a mental health condition. From the date this framework commences, frequent spot contracting will end, and this framework must be used to identify all specialist care and nursing home placements for: New clients Young People Transitioning from Children’s Services People being discharged from hospital A person with changing needs who requires a move to residential/nursing care. A person requiring an urgent placement This Framework will relate to all specialist care and nursing homes in Sheffield only, and there will be two sub-Lots. Lot 3.1 - Residential Care - Care home services without nursing Lot 3.2 - Nursing Care - Care home services with nursing The Framework includes support for the following client groups, but not limited to: Learning disabilities Autism Mental health Physical disabilities Sensory impairment Acquired brain injury Behaviours that challenge Young people transitioning into adulthood aged 16-25 These are support specialisms which SCC may need in the future. SCC will make clear what type of care or nursing home provision we require when calling off this framework.

Description: This lot includes enhanced overnight short breaks that meets the variety of needs outlined in the overarching specification. This may also include individuals with specific physical needs that may require a highly skilled or nursing element to their care. Specialised equipment & transport may be needed to meet the complex and physical needs of the individuals who will being using the service. This type of care and support will be delivered in accommodation. Therefore, any building based overnight short breaks service will require a residential or nursing home CQC registration.

Description: This lot includes enhanced emergency overnight short breaks that meets the variety of needs outlined in the overarching specification. An emergency Overnight Short Break enables the carer to have a rest from their caring role and gives a break to the person receiving support and care. The referrals made into this type of service will be at short notice, may also provide a short to medium term support and accommodation option for individuals who are in crisis and/or experiencing a breakdown in care and support. An emergency Overnight Short Break supports individuals to feel safe in an environment that will support their physical and mental wellbeing as well as meeting their needs. An emergency Overnight Short Break is a safe environment that will enable those experiencing crisis or care breakdowns to stabilise, and support them to progress to a long-term care, support and accommodation option and prevent admissions into inpatient settings.

Description: Providers on this Framework must be able to mobilise support quickly in a crisis. In some cases, this will be as quickly as a few hours up to a couple of days. Alternatively, the support may need to end at short notice, so 24 hours’ notice to end this package will be given by SCC. The provider must be able to support a wide range of support needs for people with a learning disability, autism, physical health, mental health and who display behaviours of distress. Some people may also experience severe multiple disadvantage e.g. substance misuse, looked after child, homelessness, domestic abuse, offending history. Furthermore, the provider will be expected to work with the persons MDT and attend meetings if required. There will be an expectation that providers will be able to offer PBS input. The provider will be able to provide a team of highly skilled and experienced support workers 24/7, 365 days year to meet the support needs outlined in the overarching specification. The expectation is that the support offered is temporary, and that providers on this Framework will work in partnership to successfully transition a person over to a long-term framework support provider. The support provided could be in a person’s own home or this could be provided in temporary accommodation such as in a hotel or a short term let property that the person may be living in. SCC require providers on this Framework to be CQC registered with the correct service specialisms as needed.
