PRJ 2261 - Neighbourhood Health Community Prevention and Empowerment

The South West London Integrated Care Board is inviting bids for the Neighbourhood Health Community Prevention and Empowerment Programme, a transition to integrated, locality-based health prevention and empowerment services aligned to population‑health needs across North, Central and South Croydon. The procurement is divided into three distinct lots, each targeting a specific geographical area and set of health priorities. The first lot delivers a healthy lifestyle behaviour service for North Croydon, targeting high‑risk communities through weight management, smoking cessation and alcohol reduction interventions. The second lot provides a prevention, access and health‑equity service for Central Croydon, focusing on digital inclusion for the NHS App, welfare and housing support, and childhood immunisation equity among under‑served populations. The third lot offers a combined weight management, independence and wellbeing service for South Croydon, including targeted weight‑loss support, carer‑strain mitigation and loneliness/social connection programmes for older residents.

Each lot requires the establishment of referral pathways from primary care, council services and voluntary partners, as well as collaboration with community organisations to avoid duplication of existing services. Performance will be assessed through quarterly reporting on participant demographics, engagement, completion rates and validated health outcomes, with ongoing safeguarding, governance and data‑protection arrangements. Providers may submit bids for one, two or all three lots, with separate evaluation for each lot and no cap on the number of lots that can be awarded to a single provider.

All services must comply with NHS prevention priorities, the NHS Long Term Plan, Core20PLUS5, NICE guidance and local Croydon population health priorities.

North, Central and South Croydon
South West London Integrated Care Board (SWL ICB) - Croydon Place is inviting providers, voluntary and community sector organisations, social enterprises, primary care partners and other interested organisations to submit a Tender for Croydon Neighbourhood Health Community Prevention and Empowerment Programme.
The programme represents a significant transition to integrated, locality-based neighbourhood health prevention and empowerment services aligned to population health need and the Neighbourhood Health model.
This is a single procurement exercise comprising three separate locality-based lots.
The tender is split into the following 3 Lots:
Lot 1 - SWL North Croydon Healthy Lifestyle Behaviours Service
Lot 2 - SWL Central Croydon Prevention, Access and Health Equity Service
Lot 3 - SWL South Croydon Weight Management, Independence & Wellbeing Service
Providers may submit a tender for one, two, or all three Lots. Where a Provider submits tenders for more than one Lot, each tender will be evaluated separately in accordance with the published Bid Evaluation Criteria and Weightings (Document 3) for the respective lots.
Providers will be considered for award on each individual Lot solely on the score achieved for that Lot. Please note, there is no cap on the number of Lots a single Provider can be awarded.
Procurement Specific Questionnaire is generic and applies to all Lots. It is mandatory to complete the Procurement Specific Questionnaire. In addition bidders must complete and submit Lot Specific Questionnaires for those Lots they are submitting a bid for. Failure to complete the Procurement Specific Questionnaire and at least one Lot Specific Questionnaire will result in the Bid submission being deemed non-compliant and rejected from further consideration.
The proposed contract commencement date is 1 April 2027, with an intended contract term of 3 years plus an optional 1-year extension (3+1).

Description: This service is an integrated, targeted healthy lifestyle behaviour service for high-risk communities in North Croydon, covering the North East and North West Croydon localities. It is designed to reduce preventable illness, premature mortality and health inequalities by supporting residents most at risk of poor health outcomes to make sustained changes in relation to healthy weight, smoking cessation and alcohol reduction. The service focuses on communities experiencing higher levels of deprivation, housing pressure, unemployment, limited access to healthy assets and increased cardiometabolic risk. The target population includes residents from areas of socioeconomic deprivation, Black African and Black Caribbean communities, South Asian communities, Eastern European residents, people with severe mental illness or learning disabilities, and people with long-term conditions such as hypertension and diabetes. The service will provide structured, evidence-based interventions including behavioural counselling, motivational interviewing, culturally tailored engagement, community outreach, accessible delivery locations, and digital options where appropriate. It will complement rather than duplicate existing Croydon Council universal healthy behaviour services, with clear referral and signposting arrangements between services. The three main service components are: Weight management: targeted support for eligible adults aged 25-64 with BMI above the defined threshold and additional clinical or population risk factors, with outcomes focused on programme engagement, completion and clinically meaningful weight loss. Smoking cessation: targeted support for smokers aged 40-74 with diagnosed hypertension and/or diabetes who have not used stop smoking services in the previous 12 months, with outcomes focused on treatment starts and CO-validated quit rates at 4 and 12 weeks. Alcohol reduction: targeted support for North Croydon residents aged 40-64 with diagnosed hypertension and/or diabetes and increasing or higher-risk alcohol use, with outcomes focused on brief interventions, extended support, reduced AUDIT-C scores and signposting to specialist services where dependence is indicated. The provider will be expected to establish referral pathways from GP practices, Croydon Council services, VCSE organisations and self-referrals, subject to validated eligibility evidence. The provider must also work closely with local partners, including primary care, Croydon Council, voluntary and community organisations, adult healthy behaviour services, smoking cessation services, weight management services, substance misuse providers and wider social support services. The service will be delivered under a block contract for the period 1 April 2027 to 31 March 2030, with an option to extend by one year. The annual contract value is £184,539 (Inclusive of VAT) and the total contract value is £738,156 (Inclusive of VAT), with indicative component allocations for smoking cessation, alcohol reduction and weight management. Providers may propose collaborative or consortium delivery models, provided there is a single lead contractual entity accountable for performance, governance, reporting and compliance. Performance will be monitored through quarterly reporting on participant demographics, referral source, engagement, completion, validated outcomes, onward referrals, data-sharing consent, GP outcome sharing and evidence of targeted outreach. Description: The SWL Central Croydon Prevention, Access and Health Equity Service is a targeted prevention service for Central East and Central West Croydon, running from 1 April 2027 to 31 March 2030, with an option to extend by one year. The service is designed to address health inequalities by focusing on population groups whose health outcomes are affected by digital exclusion, financial and housing insecurity, anxiety, and lower childhood immunisation uptake. The service has three main components: Central East Croydon Digital Inclusion and NHS App Uptake Programme This element supports residents aged 40 and over who are registered with a Central East Croydon GP practice, experience barriers to digital access, and are not currently registered with the NHS App. The programme will provide outreach, one-to-one support, identity verification help, and practical guidance to enable residents to use the NHS App and access online NHS services more confidently. Central West Croydon Welfare, Financial and Housing Advice Service This element targets adults in deprived areas of Central West Croydon who are experiencing financial, housing or welfare-related stress, have moderate to high anxiety, and either have a long-term condition or high primary care usage. The service will provide structured casework, debt and benefits advice, housing advocacy and financial literacy support, with the aim of reducing anxiety, improving wellbeing and addressing wider determinants of poor health. Central West Croydon Childhood Immunisation Equity Programme This element focuses on improving MMR vaccination uptake among Black and minority ethnic families with children aged 0-5 who are registered with Central West Croydon GP practices. It will use culturally competent outreach, two-way engagement, navigation support and follow-up to help families access routine immunisations. The service will be delivered through targeted outreach, community-based engagement, partnership working and flexible delivery models. Providers will be expected to work with GP practices, VCSE organisations, local authority services and other relevant partners to avoid duplication and ensure residents receive joined-up support. Key outcomes include increased NHS App registration and use, reduced anxiety scores, improved financial or housing outcomes, improved wellbeing, increased engagement with underserved families, and additional MMR vaccinations. Providers will be required to report quarterly on activity, demographics, outcomes, data quality and targeted outreach, while maintaining robust safeguarding, governance, information sharing and data protection arrangements. The annual contract value is £209,063 (Inclusive of VAT) and the total contract value is £836,252 (Inclusive of VAT). Providers may bid as a single organisation or through partnership, consortium or lead-provider arrangements, with one lead contractual entity accountable for performance and governance.

Description: The SWL South Croydon Weight Management, Independence & Wellbeing Service is a targeted prevention and empowerment service covering South East and South West Croydon. It is designed to address different local health needs through three linked service components: targeted weight management support, carer strain support, and loneliness/social connection support. In South East Croydon, the service will provide targeted weight management interventions for adults aged 25-64 who are at higher risk of poor health outcomes, including people living in more deprived areas, residents from higher-risk ethnic groups, and people with severe mental illness or learning disabilities. The aim is to support sustainable behaviour change, clinically significant weight loss, improved wellbeing, and appropriate onward referral into wider lifestyle and long-term condition support. In South West Croydon, the service will focus on supporting an older population and those affected by long-term conditions. One pathway will support unpaid carers experiencing high levels of carer strain, helping them build coping strategies, access practical and emotional support, and reduce the risk of burnout. A second pathway will support older residents experiencing loneliness and social isolation through social prescribing, group activities, befriending and personalised medium-term support planning. The service will be delivered through targeted outreach, accessible community-based provision, culturally appropriate engagement, and close partnership working with primary care, Croydon Council, voluntary and community sector organisations, and existing local services. Providers will be expected to maintain robust governance, safeguarding, information governance, reporting and outcome monitoring arrangements. The contract is proposed to run from 1 April 2027 to 31 March 2030, with an option to extend by one year.
