Services for DHHS clients Including Persons with ID.RC and/or ABI under the Limited Supports Waiver.

The Department of Health and Human Services of Utah is soliciting qualified providers to deliver attendant care, behavior supports, and employment supports for clients with intellectual disabilities (ID) and/or acquired brain injury (ABI) who are enrolled under the Limited Supports Waiver. The procurement is structured as an open‑ended Request for Statement of Qualifications (RFSQ), which will lead to the creation of an Approved Vendor List (AVL) from which services will be purchased. Vendors must submit their qualifications electronically through the procedures outlined in the RFSQ, with responses directed to the designated email address rather than the standard procurement portal.

The services to be procured comprise specialized medical and rehabilitation assistance, including speech specialist interventions and comprehensive chronic disability rehabilitation, as categorized by UNSPSC codes 85121704, 85122109, 854561, 85456107, 854562, and 85456206. Providers are expected to diagnose and address life‑management difficulties and care‑provider dependency issues among these clients, aligning with the corresponding ICD‑10 categories (Z73.x and Z74.x). All offerings must adhere to the diagnostic and service standards indicated by these classifications.
